ANSWER

– There has been a change within the necrotic cavity.
– There is a globular contrast enhanced mass representing a pseudo-aneurysm /aneurysm. Its neck measures 2mm with a maximal diameter of 1.5 cm
– The pseudo aneurysm projects into the previous necrotic cavity.
– There is still a dense consolidation in the right lower lobe with patchy parenchymal opacity in the left lower lobe and lingula.

DIAGNOSIS

The diagnosis is a pseudo- aneurysm in keeping with the angio-invasive nature of Invasive Pulmonary Aspergillosis.
