CASE


L.F. is an 8-y-old boy with Dyskeratosis Congenita, diagnosed in 2000. He had a bone marrow transplant from an unrelated donor in 2001 and a history of upper gastrointestinal bleeding April 2005 oif unknown etiology. Despite his BMT he suffers from pancytopenia.

He was admitted to a community hospital on Dec. 9,2005 with history of increased work of breathing and respiratory distress. A chesty radiograph showed right lower lobe infiltrates.

He was admitted to the Intensive Care Unit but was not intubated, and was then transferred to the Hospital for Sick Children in Toronto for further management. There he received 40% FiO2 through nasal prongs to maintain O2 Saturations of 92% or greater, and he had dips to low the 80's when he briefly stopped his supplementary O2.
